The Bergen County Board of Social Services is a Support Coordination Provider for the Division of Developmental Disabilities (DDD).  The agency provides Support Coordination for DDD eligible individuals including, but not limited to, ongoing supports with needed services such as Medicaid eligibility, Food Stamps, and Housing Options.

Support Coordinator’s will help you navigate the various services and programs that are accessible within your assigned budget. The Support Coordinator will work with you in selecting services that best meet your needs to accomplish your goals.

Support Coordinators guide, assist and support you in completing an Individualized Service Plan (ISP) that is used to monitor the delivery of services.
